(1) Use your home phone or cell phone to call "1966" to let the designated personnel to help you fill out the "Application Form for Long-term Care Services."
(2) Online application: Go to "Service E-counter" on the website site of the Taichung City Government to fill out the application form.
(3) Go to the Long-term Care Management Center of a Health Center in your district to file your application in person.
(4) Use your smartphone to download "Taichung Long-term care app."
After you file your application, our management personnel will contact you to schedule an appointment for the on-site evaluation; grants will be given according to the level of disability of the patient. The case manager will enact a care plan to provide services needed.
